Long-Term Care Ombudsman Program Grievance Policy
To file a grievance about the actions or determinations of a long-term care ombuds:
- Please read the grievance policy and procedures available [here].
- Be sure to read Sections 1 and 3 before filing a grievance about the performance of an ombuds.
- To submit a grievance, use the form available [here]. Provide as much detail as you can. Send the completed form to ltcop@mschelps.org.
Please note: A grievance is not the same as a complaint to the ombudsman program. To make a complaint to the ombudsman program about the health, safety, welfare, or rights of resident living in a long-term care facility, please click [here].
