The Grievance policy is to file a complaint about a volunteer or paid Long-Term Care Ombuds in Washington state. Please do not send complaints about facility care through this form. If you’re dissatisfied, the State Long-Term Care Ombuds has established a grievance procedure for the purpose of providing an appeal process for any individual dissatisfied with the actions of any ombudsman. Please see the following policy and procedure for instructions on how to file your formal grievance.
